Refractive Lens Exchange (RLE)

Struggling to read your phone without holding it at arm’s length? Finding yourself constantly switching between distance glasses and reading glasses?

If you’re over 40 and experiencing the frustrations of presbyopia or have been told you’re not a candidate for LASIK, refractive lens exchange (RLE) could be your solution to clear, comfortable vision without visual aids. At Longwood Eye & LASIK Center, our experienced ophthalmologists specialize in refractive lens exchange, providing patients with a proven pathway to freedom from constant dependence on glasses and contact lenses.

What is Refractive Lens Exchange?

Refractive lens exchange is an outpatient procedure that replaces your eye’s natural lens with a premium intraocular lens (IOL) designed to correct your specific vision needs. Unlike LASIK, which reshapes the cornea, RLE replaces the natural lens of the eye with a corrective IOL.

The procedure closely resembles cataract surgery but is performed before cataracts develop or in their earliest stages of development. This timing allows patients to proactively address vision changes rather than waiting for cataracts to interfere with their daily activities.

During RLE, your surgeon removes the eye’s natural lens through a micro-incision and replaces it with a carefully selected premium IOL. These advanced lenses can correct nearsightedness, farsightedness, astigmatism, and presbyopia simultaneously, often eliminating the need for reading glasses and bifocals.

Who is a Good Candidate for Refractive Lens Exchange?

RLE works particularly well for patients who have been diagnosed with presbyopia, also known as age-related farsightedness. Between the ages of 40 and 60, the eye’s natural lens begins to lose its flexibility, making it difficult to focus on close objects.

By removing the natural lens and replacing it with an artificial IOL, RLE can help you regain clear vision at near distances. You may also be a good candidate for refractive lens exchange if you:

Your eye doctor at Longwood Eye & LASIK Center will conduct a comprehensive evaluation to assess your eye health and determine your candidacy for RLE. During this consultation, they will measure your prescription, evaluate your corneal health, check for any eye diseases, and discuss your visual goals and lifestyle needs, as well as review your premium IOL options.

Premium IOLs at Longwood Eye & LASIK Center

The results of your RLE procedure depend largely on selecting the right premium IOL for your lifestyle and visual goals. Longwood Eye & LASIK Center offers the most advanced lens technologies available, including several categories of premium IOLs:

Toric IOLs

Patients with astigmatism can benefit from toric IOLs made to fit their unique eye shape and provide clear vision at one or more distances. Toric IOLs offered at Longwood Eye & LASIK Center include the Alcon Clareon Aspheric Toric, enVista Toric, and TECNIS Toric.

Extended Range of Vision (EROV) IOLs

EROV lenses offer expanded visual ranges with reduced visual disturbances. At Longwood Eye & LASIK Center, we offer the Bausch & Lomb enVista Aspire, Rayner Rayone EMV, and TECNIS Symfony EROV IOLs.

Multifocal IOLs

Multifocal lenses provide clear vision at multiple distances by incorporating different focusing zones within the lens. We feature the Alcon Clareon PanOptix Pro and Bausch & Lomb enVista Envy multifocal IOLs which offers excellent near, intermediate, and distance vision, making them ideal for patients who want to minimize glasses use for all activities.

Extended Depth of Focus (EDOF) IOLs

EDOF lenses create a continuous range of vision rather than distinct focal points and reduce visual aberrations like halos and glare. The J&J TECNIS Eyhance ICB00 and Alcon Clareon Vivity extend your depth of focus to intermediate distances while maintaining excellent distance vision quality and functional near vision.

What to Expect During Refractive Lens Exchange

The RLE procedure takes approximately 15-20 minutes per eye. Your surgeon begins by applying numbing eye drops to ensure your comfort throughout the procedure.

You may receive mild sedation if you feel anxious about the surgery. Once your eyes have been numbed, your surgeon creates a micro-incision where the cornea meets the sclera.

Ultrasonic energy gently breaks up your natural lens, which is then removed through the tiny opening. Your selected premium IOL is carefully positioned in the same location as your natural lens.

The micro-incision typically seals itself without requiring stitches. Most patients treat one eye at a time, with procedures spaced about a week apart to ensure optimal healing and visual results.

What is Recovery Like after Refractive Lens Exchange?

After surgery, you’ll rest briefly in our recovery area before going home. You’ll need someone to drive you to and from your procedure, as your vision will be temporarily blurry.

Most patients notice visual improvements within 24-48 hours after RLE. Your eyes continue adapting to the new lenses over several weeks, with vision typically stabilizing within a month. During recovery, you’ll use prescribed eye drops to promote healing and prevent infection.

Your post-operative care includes avoiding rubbing your eyes, keeping water out of your eyes while showering, wearing protective eyewear while sleeping, and limiting strenuous activities for the first week. Most patients return to normal activities within a few days and can resume driving once they have been cleared by their eye surgeon.

If you’re tired of the limitations that presbyopia and refractive errors place on your daily life, refractive lens exchange at Longwood Eye & LASIK Center could provide the visual freedom you’ve been seeking. Our advanced Premium IOL technology and commitment to compassionate, personalized patient care ensure you receive the best possible outcomes.
