What is Refractive Lens Exchange?

Refractive lens exchange is an outpatient procedure that replaces your eye’s natural lens with a advanced lifestyle lens designed to correct your specific vision needs. Unlike LASIK, which reshapes the cornea, RLE replaces the natural lens of the eye with a corrective lenses. The procedure closely resembles cataract surgery but is performed before cataracts develop or in their earliest stages of development. This timing allows patients to proactively address vision changes rather than waiting for cataracts to interfere with their daily activities. During RLE, your surgeon removes the eye’s natural lens through a micro-incision and replaces it with a carefully selected advanced lifestyle lenses. These advanced lenses can correct nearsightedness, farsightedness, astigmatism, and presbyopia simultaneously, often eliminating the need for reading glasses and bifocals.

Who is a Good Candidate for Refractive Lens Exchange?

RLE works particularly well for patients who have been diagnosed with presbyopia, also known as age-related farsightedness. Between the ages of 40 and 60, the eye’s natural lens begins to lose its flexibility, making it difficult to focus on close objects.

By removing the natural lens and replacing it with an artificial lens, RLE can help you regain clear vision at near distances. You may also be a good candidate for refractive lens exchange if you:

  • Have been told you’re not a good candidate for LASIK due to age, thin corneas, dry eye, or severe refractive errors
  • Want to reduce your dependence on glasses and contacts
  • Want to treat early cataracts or prevent cataracts from forming in the future
  • Have had a stable prescription for at least one year or more
  • Are not pregnant or nursing
  • Do not have eye conditions like glaucoma, age-related macular degeneration, or uveitis
  • Do not have autoimmune conditions like lupus, rheumatoid arthritis, or Sjogren’s syndrome
  • Have realistic expectations about RLE outcomes

What to Expect During Refractive Lens Exchange

The RLE procedure takes approximately 15-20 minutes per eye. Your surgeon begins by applying numbing eye drops to ensure your comfort throughout the procedure. 

You may receive mild sedation if you feel anxious about the surgery. Once your eyes have been numbed, your surgeon creates a micro-incision where the cornea meets the sclera. 

Ultrasonic energy gently breaks up your natural lens, which is then removed through the tiny opening. Your selected advanced lifestyle lens is carefully positioned in the same location as your natural lens.

The micro-incision typically seals itself without requiring stitches. Most patients treat one eye at a time, with procedures spaced about a week apart to ensure optimal healing and visual results.

What is Recovery Like after Refractive Lens Exchange?

After surgery, you’ll rest briefly in our recovery area before going home. You’ll need someone to drive you to and from your procedure, as your vision will be temporarily blurry.

Most patients notice visual improvements within 24-48 hours after RLE. Your eyes continue adapting to the new lenses over several weeks, with vision typically stabilizing within a month. During recovery, you’ll use prescribed eye drops to promote healing and prevent infection.

Your post-operative care includes avoiding rubbing your eyes, keeping water out of your eyes while showering, wearing protective eyewear while sleeping, and limiting strenuous activities for the first week. Most patients return to normal activities within a few days and can resume driving once they have been cleared by their eye surgeon.
